伤寒，其脉微涩者，本是霍乱，今是伤寒，却四五日至阴经，阳转入阴，必利，本呕下利者，不可治也。欲似大便而反失气，仍不利者，此属阳明也，便必鞭，十三日愈。所以然者，经尽故也。
伤寒之症如脉微涩者，这是四逆汤证，病家是因为心阳不振，导致寒水不能气化造成血寒于体内，病原是 自霍乱，而今有伤寒之症，因此用麻黄汤之类无法治好，失治后三周左右，病由阳入太阴，就会下利，此时还有上热未去，无法给予热药，故不可施治。等到阳热自回后，大便转燥而利止，即排出矢气，这时就是阳明证，其便必硬，二周内会好，这是因为阳明经尽止于二周。

下利后，当便鞭，鞭则能食者愈。今反不能食，到后经中颇能食，复过一经能食，过之一日当愈。不愈者，不属阳明也。
下利之症如转热后，大便必硬。如果大便硬而胃口好，这是回到阳明证，表示自己会痊愈的，不需要治。但是如果病人没有胃口，就要等十三日，之后病人能开始吃食物以后就会好了。如果没好，就表示病不在少阳与阳明，可能已经入里了。
利止，恶寒脉微，而复利，亡血也，四逆加人参汤主之。

如果下利停止，病人又出现恶寒脉微小无力，之后又开始下利时，就表示病人血寒了，此时救逆的处方就是四逆加人参汤了。四逆汤是温阳救逆第一方，功能起死回生，多使用于心脏衰竭之危症时。此处之失血，并不是真的贫血，而是血寒，所以不用当归、川芎、白芍之类的补血药物。
